As part of the new contract, a Medical, Vision and Dental cost share will be implemented on July 1st 2010.

 

The cost share amounts are as follows:

 

Single:              $50/ month / (approx $11.53 wk)

 

Two people:     $110/ month / (approx $25.38 wk)

 

Family:             $175.00/ month / (approx $40.38 wk)

  

Since adding a cost share is a significant change the Company is allowing employees to make changes to their covered dependents.

 

• To remove dependents you will need to fill out a Family Status Change form.

 

• To add a dependent who is another Daimler Trucks North America employee you will need to fill out a Family Status Change form (Note: you will need a form to remove them from their current Daimler Trucks North America benefits plan and one to add them to your Daimler Trucks North America benefits plan).

 

You can find Family Status Change form at www.truckbenefits.com , call ADP at 1-800-605-6031 or pick one up in the Main lobby.  On the form please write in other cost share as the reason for the change.
